THE FOLLOWING ITEMS ARE NEEDED FOR COMPLETION OF THE LOAN PACKAGE FOR BOTH THE BOARD AND SBA:

  1. Map of project location

  2. History and Description of the Business (new or existing)

  3. SBA Form 912 - Personal History and Description - completed for each person owning 20% or more of the business

  4. Breakdown of officers and percentage of ownership

  5. Personal Financial Statement of all borrowers current within 90 days

  6. The past three years of personal income tax returns

  7. Signed Policy/Consent Form

  8. Resumes of the principals

  9. The past three years of business financial statements (accountant prepared)

  10. The past three years of business tax returns

  11. Interim business financial statement current within 90 days

  12. Two years of proforma financial statements with cash flow statements and assumptions

  13. List of business obligations

  14. Name of affiliate(s) with historicals and interim statements

  15. Affiliate’s income tax returns for the past three years

  16. Bank Commitment Letter

  17. Schedule of previous government financing on anyone involved with the loan

  18. Franchise Agreement and Franchiser’s Disclosure Statement by FTC

  19. Cost estimates and useful life statements
    1. construction contract must have total dollar amount and be signed by the contractor
    2. each vendor must provide a quote for any equipment to be purchased with a ten year useful life statement attached
    3. land purchase agreement or settlement statement

  20. "As-built" real estate appraisal

  21. Environmental questionnaire or Phase I Audit with reliance letter addressed to SBA

  22. Articles of Organization and Bylaws/Operating Agreement (signed)
